BRAINS RULE! NEUROSCIENCE CYBEREXPOSITION

June 2004

Professional Participant Questionnaire

The Brains Rule! project is funded through an R25 Training Grant from NIDA. In order to evaluate the outcome of this project, we need you to participate and help us in our data collection. Please take a few minutes to fill out the following questions. Information gathered through this questionnaire will be kept strictly confidential and no identifiers attached. Sometime in the future, after you have been participating as a Brain Whiz, we will send a second questionnaire. Data from the first set of responses and the second set are pooled (and not linked pre- and post) so responses remain totally anonymous.

Thank you for the extra time you will be investing in this effort to complete these surveys!
